Job summary

Outfield Healthcare Partners seeks a qualified MDS Coordinator in Missouri to manage timely and accurate completion of the RAI and care management process from admission to discharge, in alignment with policy and federal/state guidelines.

You will lead interdisciplinary team efforts, monitor MDS coding, maintain records, and ensure regulatory compliance while coordinating schedules and educating staff on the MDS process.

Qualifications

• Graduate of an approved RN program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by governing agencies.
• Decide with the Interdisciplinary team whether to complete additional MDS assessments based on clinical criteria per the RAI User’s Manual.
• Coordinate daily stand-up meetings including review of resident care and assessment reference dates.
• Comply with federal and state regulations regarding completion and coordination of the RAI process.
• Monitor MDS and care plan documentation in the medical record to support MDS coding.
• Maintain current MDS status of assigned residents per state and federal guidelines.
• Maintain frequent and accurate data entry of resident information into MDS programs.
• Complete accurate coding of MDS with data from records, observations, and interviews.
• Attend interdisciplinary meetings to gather information and update records.
• Assist DON with identification of significant changes, physician orders, and verbal reports to reflect changes in MDS and care plans.
• Prepare scheduling and notices of care planning conferences; communicate outcomes to staff and residents.
• Continually update knowledge base related to data entry and computer technology.
• Submit electronic documentation to state databases and other entities per policy.
• Correct and finalize MDS and submit data to state/federal agencies.
• Assign, instruct staff in the RAI Process, PPS Medicare, Medicaid, and clinical systems.
• Maintain confidentiality of necessary information.
• Other duties as assigned.
