Job summary

Vertical Health Services in Missouri is seeking an experienced MDS Coordinator (RN/LPN) to oversee the facility's RAI and Medicare processes. You will verify resident eligibility, ensure complete documentation for continuing stay, and manage timely MDS/PPS assessments in compliance with regulations.

This full-time position offers 12-hour shifts, every other weekend, rotating holidays, and a comprehensive benefits package, with sign-on bonus for eligible candidates.

Qualifications

• Graduate of an approved Nursing program and licensed in the state you are applying
• 18 months of recent experience as MDS Coordinator
• Experience with clinical reimbursement
• Current BLS/CPR certification
• Reliable transportation

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews
• Coordinate daily stand-up meetings and review resident care and assessment dates
• Monitor MDS and care plan documentation in the medical record to support coding
• Identify significant changes with DON or designee and update MDS/care plans accordingly
• Ensure completion and timeliness of RAI processes from MDS to care plan
• Report trends from audits to QA Committee
• Maintain confidentiality of necessary information
• Other duties as assigned
