Job summary

Elevance Health is seeking a LTSS Service Coordinator to manage a defined caseload in specialized programs, conducting person centered planning, assessments and developing a comprehensive PCSP with a backup plan. You will interface with Medical Directors and the interdisciplinary care team to support integrated care.

The role involves documenting member goals with their care team, coordinating with caregivers and service providers, and addressing LTSS/BH/PH needs in compliance with state

Qualifications

• BA/BS degree required.
• Minimum of 2 years of experience with a social work agency, or equivalent background.
• Education/experience requirements may vary by state law and contract.

Responsibilities

• Conduct face-to-face program assessments using predefined questions and motivational interviewing to identify needs.
• Identify members with potential clinical health care needs and coordinate cases with the interdisciplinary team.
• Manage non-clinical needs of members to ensure cost-effective, efficient LTSS use.
• Document member goals in collaboration with the chosen care team (family, caregivers, providers).
• Identify members who may benefit from alternative services or other waiver programs.
• May mentor or train new staff and contribute to process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies as required.
• Assist with appeals, grievances, and state audits as needed.
