Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• Bilingual in English/Spanish strongly preferred.
• LTSS, case management, social work or hospital discharge planning experience preferred.

Responsibilities

• Conduct face-to-face program assessments for identification, using predefined questions and motivational interviewing techniques.
• Coordinate and manage an individual’s LTSS/IDD and BH or PH needs with the clinical care team as the single point of contact.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective utilization of long-term services and supports.
• Document short and long-term service goals with the member and care team, including caregivers and providers.
• Identify members who may benefit from alternative levels of service or other waiver programs.
• May mentor or train new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with documentation aligned to the care plan.
• Report critical incidents to state/county agencies as required and participate in state audits.
