Job summary

Elevance Health is seeking an LTSS Service Coordinator to operate primarily in the field with travel to client sites and occasional office attendance. The role focuses on coordinating LTSS/IDD services, developing Person Centered Plans, and collaborating with Medical Directors and care teams to support integrated health needs.

The position requires a BA/BS and a minimum of two years in a social work capacity, with flexibility for locations within commuting distance.

Qualifications

• BA/BS degree required.
• Minimum of 2 years of experience with a social work agency or equivalent background.
• Education and experience may be adjusted based on state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments using predefined questions and motivational interviewing.
• Coordinate and manage an individual's waiver (LTSS/IDD), BH or PH needs within a defined caseload.
• Document and develop a Person Centered Support Plan (PCSP) and backup plan with the member and care team.
• Engage the member's circle of support and care providers to develop an integrated care plan.
• Identify members who may benefit from alternative levels of service or other waiver programs.
• May mentor or train new staff and contribute to process improvements.
• Submit utilization/authorization requests with appropriate documentation.
• Report critical incidents to state and county agencies as required.
• Support member grievances, appeals and state audits.
