Job summary

Elevance Health is seeking a Field-based LTSS Service Coordinator Clinician to work under the supervision of an RN, coordinating care across physical and behavioral health needs. This role travels to client sites or designated locations, with occasional office attendance as needed.

Minimum requirements include licensure (LPN/LVN or SW licenses) and at least 2 years of experience with chronic illnesses or disabilities.

Qualifications

• Must hold LPN/LVN, LSW, LCSW or LMSW license as required by state law.
• Minimum of 2 years working with individuals with chronic illnesses or disabilities in a service coordination or similar role.
• Current, unrestricted license as LPN/LVN, LSW, LCSW, LMSW or RN-eligible license in applicable state(s).
• State-specific certification may be required by contract.

Responsibilities

• Assist the responsible RN in identifying members for high risk situations.
• Collect clinical data as directed by the RN.
• Help identify members that could benefit from alternative levels of care or waiver programs.
• Support the RN in developing, monitoring and revising care plans to meet member needs.
