Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician in Indiana to support field-based care coordination for members with chronic illnesses and functional needs. You will work under the supervision of an RN, performing assessments and contributing to care plans across physical and behavioral health domains.

Preferred candidates hold MA/MS in social work and licensure (LSW/LCSW/LMSW) with 2+ years in similar roles.

Qualifications

• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) required.
• Minimum of 2 years experience with individuals with chronic illnesses/ disabilities in a Service Coordinator or similar role.
• Experience working under RN supervision and contributing to LTSS care coordination.

Responsibilities

• Assist the RN in identifying high-risk members and collecting clinical data.
• Help identify members who may benefit from alternative levels of care or waiver programs.
• Support development and monitoring of each member's care plan with the health team.
• Coordinate care for members with chronic illnesses and disabilities to optimize health benefits and costs.
• Communicate findings to the RN for interpretation and further action.
