Job summary

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) in Wisconsin to support the Family Care program. This field-based role requires travel to client sites and occasional office attendance, with hybrid work considerations and a focus on person-centered planning and integrated care.

You will assess needs, develop comprehensive PCSPs, and participate in interdisciplinary rounds to ensure coordinated and quality care for members with PH/BH/LTSS needs.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency or equivalent background.

Responsibilities

• Perform face-to-face program assessments using predefined tools and questions.
• Coordinate and manage a member's waiver and health needs in collaboration with the care team.
• Document member goals and care plans with their care team and circle of support.
• Support and participate in training for new staff and process improvement initiatives.
• Submit utilization requests with documentation aligned to the care plan.
• Report critical incidents to appropriate internal and external parties.
