Job summary

UnitedHealth Group is seeking a LTSS Case Manager to coordinate care for members in Colorado, with a focus on intake, assessment, care planning, and ongoing monitoring. You will document activities in the prescribed system, ensure confidentiality, and participate in case conferences with supervision.

You’ll travel up to 75% within the territory, visiting members in homes or long-term care facilities. Remote work is possible within listed counties, and you must be flexible and patient.

Qualifications

• Bachelor’s degree in human behavioral science fields or equivalent.
• At least 5 years of LTSS experience or an education/experience mix.
• Proficiency with MS Office (Word, Excel, Outlook).

Responsibilities

• Intake, screening, referrals, assessments, and care plan development.
• Maintain confidentiality per policy and CMA contract requirements.
• Make quarterly contact with members and conduct face-to-face visits every six months.
• Complete annual reassessments and update documentation accordingly.
• Monitor service delivery and PAR alignment with the support plan.
• Attend CMA and internal training sessions.
• Travel as required; reliable transportation and CO residency near listed counties.
• Escalate suspected abuse or neglect to protective services or law enforcement.
• Engage in special assignments or system development projects as needed.
