Job summary

UnitedHealthcare is seeking a Case Manager specialized in Long Term Services and Supports. You will conduct intake, assessments, and develop care plans, monitoring health and welfare of members in their homes or long-term care facilities.

The role requires travel up to 75% within assigned territory; remote work is possible within Lake, Chaffee, Fremont or Custer Counties, CO. You will collaborate in case conferences and ensure confidentiality per policy.

Qualifications

• Bachelor’s degree or 5+ years in LTSS; combination accepted.
• 1+ year of MS Office experience (Word, Excel, Outlook).
• Must reside locally to Lake, Chaffee, Fremont or Custer Counties, CO with ability to travel up to 75%.

Responsibilities

• Intake/screening, assessment, and care plan development in the LTSS program.
• Maintain confidentiality and document case management activities in prescribed systems.
• Conduct quarterly and face-to-face contacts per program rules; update assessments.
• Complete annual reassessment and monitor service delivery per the care plan.
• Coordinate tracking of services and PARs; ensure timely, accurate work.
• Participate in CMA trainings and fulfill CMA contract requirements.

Education

Bachelor's degree in one of human behavioral science fields such as human services, social work, psychology, or public health
