Job summary

Responsibilities include quarterly/biannual in-person contacts, documentation, and participation in training. Travel up to 75% within the assigned territory is required; MS Office proficiency is expected.

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of LTSS experience (or combination of education and experience).
• Proficiency with MS Office (Word, Excel, Outlook).

Responsibilities

• Intake, assessment and development of care plans; ongoing case management.
• Monitor member health and welfare; document activities in prescribed systems.
• Conduct quarterly and six-month face-to-face contacts and reassessments.
