Job summary

UnitedHealthcare is seeking a Long Term Services and Supports (LTSS) Case Manager in Colorado. The role focuses on intake, assessments, care planning, and ongoing case management for members in home or long-term care settings.

Expect up to 75% travel within the assigned territory and potential remote work for Mesa County, CO. The position emphasizes confidentiality, policy adherence, and collaboration with a supervisor and CMS guidelines.

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of LTSS experience.
• Combination of education and experience.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of health and welfare, documentation in prescribed system, resource development, and case closure.
• Maintain confidentiality of patient information as described in the employee handbook.
• Conduct monitoring contacts at least quarterly or as warranted by LTSS program.
• Face-to-face contact with the member at least every six months or as required by LTSS program; update assessment tool and care plans accordingly.
• Complete a new assessment during annual reassessment or more frequently if needed.
• Monitor delivery of services identified in the Support Plan and PAR.
• Attend Department-provided CMA training and in-service.
• Refer suspected mistreatment to protective services or local law enforcement immediately.
• Take on special assignments and assist with system assessment and development as needed.
• Travel is required; independent transportation and Colorado licensure requirements apply.
