Job summary

Qualifications

• Bachelor's degree in human behavioral science fields or equivalent.
• 5+ years of LTSS experience or combination of education and experience.

Responsibilities

• Intake, assessment, and development of support plans; ongoing case management and documentation.
• Confidentiality and adherence to policies; conduct quarterly and six-monthly contacts with members.
• Annual reassessments, monitor service delivery, and ensure compliance with CMA contracts.
• Travel within assigned territory up to 75% to meet with members and providers.
• Participate in training and case conferences; handle complex situations and protective referrals as needed.
