Job summary

UnitedHealthcare in Colorado is seeking a LTSS Case Manager to support home-based and facility-based members through intake, assessments, and care planning. You will document activities and monitor health outcomes while coordinating with supervisors and care teams.

The role requires confidentiality, travel up to 75%, and the ability to work remotely within Mesa County. You will participate in care conferences and ensure timely updates to plans and records.

Qualifications

• Bachelor's degree in human behavioral sciences such as human services, social work, psychology, or public health.
• 5+ years of LTSS experience or combination with education.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Reliable transportation and travel up to 75% within assigned territory.
• Local to Mesa County, CO.

Responsibilities

• Intake, screening, referral, assessment, reassessment, and ongoing case management.
• Maintain confidentiality of patient information and document activities in prescribed system.
• Monitor member health and welfare; update plans as conditions change.
• Face-to-face contacts with members at least every six months; annual reassessments.
• Travel between locations; attend CMA trainings and maintain timely documentation.
