Job summary

UnitedHealthcare is seeking a Long Term Services and Supports (LTSS) Case Manager in Colorado to coordinate intake, assessments, and ongoing support for members, including developing care plans and monitoring health and welfare.

The role requires annual face-to-face reassessments, regular member contact, and collaboration with supervision. Remote work flexibility is offered with travel within the territory as needed, plus a comprehensive benefits package.

Qualifications

• Bachelor's degree in a human behavioral science field or related experience in LTSS
• 1+ years of experience with MS Office (Word, Excel, Outlook)
• Reliable transportation and ability to travel up to 50% within the territory to meet with members and providers
• Local residence to Alamosa, CO and surrounding counties

Responsibilities

• Intake, assessment, development of support plans, ongoing case management and documentation
• Maintain confidentiality and follow CMA program requirements
• Quarterly and six-month member contacts; annual reassessments
• Update assessment tools and care plans reflecting changes in condition
• Attend CMA trainings and participate in program-related activities
• Travel between locations as required by the job

Education

Bachelor's degree in human behavioral science fields such as human services nursing social work psychology public health
