Job summary

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of LTSS experience.
• Combination of education and experience is acceptable.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 75% within the territory.
• Reside locally to Mesa County, CO.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management.
• Monitor health and welfare, document contacts and activities in the prescribed system.
• Conduct quarterly and six-monthly face-to-face contacts with members; update assessments and care plans.
• Complete annual reassessments and additional assessments as needed.
• Ensure services adhere to CMA contract requirements and report any suspected mistreatment.
• Travel between locations; meet Colorado licensure and insurance requirements for driving.
