Job summary

UnitedHealthcare is seeking a Case Manager to support Long Term Services and Supports (LTSS). The role involves in-home visits, assessments, care planning, and ongoing coordination to ensure quality member care.

Responsibilities include intake, reassessments, monitoring, and documentation, with travel up to 75% within the territory. Local Mesa County, CO residency preferred with potential remote work options for eligible candidates.

Qualifications

• Bachelor's degree in human behavioral science fields or 5+ years LTSS experience.
• Access to reliable transportation and ability to travel up to 75% within assigned territory to meet with members and providers.
• Reside locally to Mesa County, CO.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of the Member's health and welfare, documentation in the prescribed system, resource development, and case closure.
• Maintain confidentiality of patient information and follow the Employee Handbook guidelines.
• Conduct monitoring contacts at least once per quarterly period and more frequently as warranted.
• Conduct face-to-face contacts with the Member at least every six months, updating assessment tools and care plans as needed.
• Complete new assessments during face-to-face reassessments annually or more frequently if warranted.
• Monitor service delivery identified within the Support Plan and PAR.
• Attend Department training for CMA agencies and in-service trainings.
• Ensure work aligns with CMA Contract requirements for accuracy and timeliness.
• If mistreatment or safety concerns arise, refer to protective services or local law enforcement immediately.
• May take on special assignments and assist with system assessment and development.
