Job summary

UnitedHealthcare is seeking a Case Manager to support Long Term Services and Supports (LTSS). You will perform intake, assessments, and ongoing case management, visiting members in their homes or in long-term care facilities.

The role requires travel up to 75% within Colorado and remote work flexibility within assigned counties. Responsibilities include developing care plans, monitoring services, maintaining confidentiality, and participating in program trainings.

Qualifications

• Must have a Bachelor's degree in human behavioral science fields (human services, social work, psychology, or public health).
• 5+ years of LTSS experience or a combination of education and experience.
• 1+ year of MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 75% within assigned territory.
• Reside locally to Lake, Chaffee, Fremont or Custer Counties, CO.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of member health and welfare, and documentation.
• Maintain confidentiality as described in the employee handbook and ensure protection of patient information.
• Conduct quarterly and biannual contacts and face-to-face assessments as required by LTSS program rules.
• Update assessments and care plans in the documentation system reflecting condition or service changes.
• Travel between locations; ensure timely arrival and departure; comply with Colorado licensure and liability insurance requirements.
