Job summary

UnitedHealth Group is seeking a dedicated LTSS Case Manager to support home and community-based services. You will perform intake, assessments, and development of care plans, coordinating services for members in their homes or long-term care facilities.

The role requires local Colorado residence, travel within the territory, and strong documentation skills. You will collaborate with supervisors, attend trainings, and ensure confidentiality and compliance across all activities.

Qualifications

• Bachelor's degree in human behavioral sciences fields (human services, social work, psychology, or public health).
• 5+ years of relevant LTSS experience or a combination of education and experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Must reside locally to Moffat County, CO or surrounding areas and have reliable transportation.

Responsibilities

• Lead intake, assessment, and development of care plans; document case management activities.
• Maintain confidentiality in line with policies and HIPAA requirements.
• Conduct quarterly and six-month face-to-face contacts with members and update assessments.
• Perform annual reassessments and adjust care plans as needed.
• Monitor service delivery and approve PARs for services.
• Attend CMA trainings and participate in required in-service sessions.
• Travel within territory to visit members and providers as needed.
