Qualifications

• Bachelor's degree in human services, social work, psychology, or public health.
• 5+ years of LTSS experience.
• 1+ year experience with MS Office (Word, Excel, Outlook).
• Reliable transportation; travel up to 50%.
• Local residency near Moffat County, CO; driver's license.

Responsibilities

• Intake, assessment, and development of support plans; ongoing case management.
• Maintain confidentiality per policies and CMA guidelines.
• Monitor contacts quarterly or as needed to support member.
• Face-to-face member contacts at least every six months; document changes.
• Annual reassessment and updates to care plans in documentation system.
• Monitor service delivery and PAR compliance.
• Attend CMA agency training and in-service sessions.
• Perform work per CMA Contract requirements; accurate and timely.
• Refer suspected mistreatment to protective services immediately.
• Take on special assignments and system development tasks as needed.
• Travel between locations; meet Colorado licensure and insurer requirements.
