Job summary

UnitedHealthcare is seeking a dedicated Case Manager for LTSS in Colorado. You will perform intake, assessments, and ongoing case management, ensuring timely documentation and care plan updates. Travel across assigned counties is required and certain trainings will be provided by CMA.

The role emphasizes confidentiality and adherence to program rules. Candidates should have a Bachelor's in a human behavioral field or equivalent LTSS experience, plus proficiency with MS Office and reliable

Qualifications

• Bachelor's degree in human services, nursing, social work, psychology, or public health.
• 5+ years of LTSS experience or an education/experience combination.
• Proficiency with MS Office tools.

Responsibilities

• Intake/screening/referral, assessment, development of support plans and ongoing case management.
• Maintain confidentiality and comply with program rules.
• Conduct quarterly contact and face-to-face visits with Members.
• Update assessments and care plans in documentation systems.
• Monitor service delivery per Support Plan and PAR.
• Attend CMA training and in-service sessions.
• Travel between locations as required, at incumbent's expense.
