Job summary

UnitedHealthcare in Colorado's Delta region seeks a LTSS Case Manager to perform intake, assessments, care planning, and ongoing member support in home or facility settings. Travel up to 50% within the territory is required.

Hybrid work in the Delta office two days a week; requires a Bachelor's or 5+ years LTSS, MS Office proficiency, and a valid driver's license; competitive hourly pay from $27 to $40 plus benefits.

Qualifications

• Bachelor’s degree in human behavioral sciences or related field, or equivalent combination of education and experience.
• 5+ years of LTSS experience or a combination of education and experience.
• Proficiency with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 50% within the territory.
• Driver's license and local residency near Delta, CO.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of health and welfare, and documentation.
• Maintain confidentiality and comply with the employee handbook and privacy requirements.
• Conduct quarterly contact with members and face-to-face visits at least every six months; update assessments and care plans as needed.
• Complete annual reassessments and monitor service delivery and prior authorizations.
• Attend CMA training and participate in in-service and staff development.
• Travel between locations; ensure timely, accurate, and complete work.
• Report suspected abuse or mistreatment to the protective services or local authorities.
• May take on special projects and assist with system assessment and development.
