Job summary

UnitedHealthcare is hiring a LTSS Case Manager to support home and long-term care members in Colorado. The role involves intake, assessment, care planning, and ongoing case management, with travel within the assigned territory and hybrid work in the Delta office two days per week.

You will conduct face-to-face contacts, maintain confidential documentation, and participate in required CMA trainings while ensuring compliance with program rules and the department's policies.

Qualifications

• Bachelor's degree in human behavioral sciences such as human services, social work, psychology, or public health.
• 5+ years of relevant experience in LTSS.
• Combination of education and experience accepted.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of health and welfare.
• Maintain confidentiality of patient information and follow the UHG Employee Handbook requirements.
• Conduct monitoring contacts at least once per quarter and more frequently as warranted.
• Conduct face-to-face contact with members at least every six months and update documentation accordingly.
• Complete a new assessment during reassessments annually or as required by program rules.
• Monitor service delivery within the Support Plan and Prior Authorization Request (PAR).
• Attend CMA agency training and in-service sessions to stay current.
• Ensure work complies with the CMA Contract Agreement requirements (accuracy, timeliness, completeness).
• If abuse/mistreatment suspected, refer to protective services and law enforcement immediately.
• May take on special assignments and projects and assist with system assessment as needed.
• Travel is required; must have independent transportation and Colorado license requirements.
