Job summary

UnitedHealthcare is seeking a dedicated LTSS Case Manager to support members in Colorado, applying assessments, care plans, and ongoing monitoring to ensure quality care. This role involves home visits, collaboration with the Supervisor, and ensuring confidentiality in all member interactions.

The position requires 50% travel within the territory, with hybrid work in the Delta office (2 days/week). A bachelor’s degree or equivalent LTSS experience is required, plus MS Office proficiency.

Qualifications

• Bachelor's degree in a human behavioral science field or public health.
• Minimum 1 year of MS Office experience (Word, Excel, Outlook).
• Access to reliable transportation and willingness to travel up to 50% within the territory.
• Reside locally to Delta, Hinsdale, or Gunnison County, CO (or surrounding areas).
• Driver's license and ability to travel as required.

Responsibilities

• Perform intake/screening/referral, assessments, and care plan development; monitor health and welfare.
• Maintain confidentiality per policy and preserve patient information.
• Contact members quarterly and conduct face-to-face reviews at least every six months.
• Update assessments and care plans in the documentation system as conditions change.
• Complete annual reassessments and adjust services accordingly.
• Oversee service delivery against the care plan and PAR; participate in training.
