Job summary

UnitedHealth Group is seeking a case manager for LTSS in Colorado with strong assessment, planning, and documentation skills. The role requires travel and diligent coordination with caregivers and providers to ensure high-quality member services.

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of relevant LTSS experience or combination of education and experience.
• Access to reliable transportation and ability to travel up to 50% within assigned territory.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of the Member's health and welfare, and documentation of contacts and case management activities.
• Maintain confidentiality of patient information per the Employee Handbook and sign-off requirements.
• Conduct monitoring contacts with Members quarterly or as warranted by condition and LTSS program rules.
