Job summary

UnitedHealthcare seeks a LTSS Case Manager in Colorado (Delta, Hinsdale, or Gunnison County) to coordinate care, assess needs, and develop support plans. The role includes home visits, quarterly contacts, and annual reassessments, with travel required and a hybrid work arrangement in the Delta office two days a week.

Qualifications

• Bachelor’s degree in human behavioral sciences or related field or 5+ years LTSS experience.
• 1+ years MS Office (Word, Excel, Outlook).
• Ability to travel up to 50% within assigned territory and reside locally to Delta, Hinsdale, or Gunnison County, CO.

Responsibilities

• Intake, screening, referral, assessment, and development of support plans for members.
• Conduct quarterly and annual face-to-face contacts and reassessments.
• Monitor service delivery and ensure adherence to the LTSS program rules and CARE plans.
• Maintain confidentiality and comply with CMA contracts and Department requirements.
• Travel to member homes and facilities; participate in trainings and CMA-related activities.
