Job summary

UnitedHealthcare is seeking a Long Term Services and Supports Case Manager to perform intake, assessment, and ongoing case management for members in Colorado. The role combines home visits with in-office responsibilities, requiring up to 50% travel within the Delta region.

You will monitor health and welfare, maintain documentation, and participate in quarterly/annual reassessments while ensuring confidentiality and adherence to program rules. Hybrid work arrangement available.

Qualifications

• Bachelor’s degree in human behavioral sciences fields such as human services, social work, psychology, or public health.
• Proficiency with MS Office (Word, Excel, Outlook).
• Willingness to travel up to 50% within assigned territory.
• Reside locally to Delta, CO or surrounding areas.
• Driver's license and reliable transportation.

Responsibilities

• Intake/screening, assessment, development of support plans and ongoing case management.
• Monitor member health and welfare; document case activities in the prescribed system.
• Conduct quarterly and semi-annual face-to-face contacts with members; update assessments as needed.
• Attend CMA training and ensure compliance with contract requirements.
• Travel within the territory to visit members and providers.
