Job summary

UnitedHealthcare is seeking a case manager for LTSS to provide in-home and facility-based assessments, develop and monitor care plans, and coordinate services for members in Delta, CO. The role requires travel and flexibility in a hybrid Delta office arrangement.

The position emphasizes confidentiality, quarterly or more frequent member contact, and annual assessments, with training provided. Local residency in Delta/Hinsdale/Gunnison CO areas is required, plus a valid driver's license.

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health; or 5+ years of LTSS experience; or a combination of education and experience.
• Minimum 1+ year of experience with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 50% within assigned territory to meet with members and providers.
• Reside locally to Delta, Hinsdale, or Gunnison County, CO or surrounding areas.
• Driver's License and access to reliable transportation.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans and ongoing case management.
• Monitor member's health and welfare and document activities in the prescribed system.
• Conduct quarterly/more frequent contact and face-to-face visits at least every six months.
• Perform annual reassessments and update care plans accordingly.
• Monitor service delivery and Prior Authorization Requests (PAR).
• Attend CMA training and agency-provided in-service sessions.
• Adhere to CMA Contract Agreement requirements and ensure timely, accurate work.
• Refer suspected mistreatment to protective services or law enforcement as needed.
• Take on special assignments and assist with system assessment and development.
