Qualifications

• 5+ years of LTSS experience or equivalent combination.
• Proficient with MS Office (Word, Excel, Outlook).
• Reliable transportation and up to 75% travel.
• Residency near Lake, Chaffee, Fremont or Custer Counties, CO.

Responsibilities

• Intake, screening, referrals; assess and reassess; develop support plans; ongoing case management.
• Monitor member health, document activities in required systems; develop resources; close cases.
• Maintain confidentiality; conduct quarterly and face-to-face contacts as required.
• Complete annual reassessments; update plans reflecting changes in condition or services.
• Oversee service delivery against the care plan and PAR; participate in CMA activities.
• Attend Department training; perform work in line with contract requirements.
