Job summary

UnitedHealthcare Colorado seeks a Case Manager specializing in Long Term Services and Supports to perform intake, assessment, development of support plans, and ongoing case management for members. You will participate in case conferences with the Supervisor and uphold confidentiality as required by policy.

Based in the Alamosa area, there is flexibility to work remotely within the region, with travel up to 50% to visit members and providers.

Qualifications

• Bachelor's degree in human behavioral science fields or equivalent.
• 5+ years of LTSS experience or combination with education.
• 1+ years of MS Office experience.
• Local residence in the Alamosa, CO area and travel up to 50%.

Responsibilities

• Intake, assessment, care plan development, and ongoing case management.
• Monitor member health, document in the prescribed system.
• Attend CMA trainings and ensure compliance with CMA contracts.
• Travel to member locations as needed for visits and assessments.
