Job summary

UnitedHealthcare is seeking a Case Manager for LTSS to conduct intake, assessments, and ongoing case management. The role includes home visits across assigned territory, development of support plans, and monitoring patient health under a confidential, compliant framework.

The position requires a degree in human services or related field, 5+ years LTSS experience or equivalent, and travel up to 75%. Remote work is possible for residents in Lake, Chaffee, Fremont or Custer Counties, CO.

Qualifications

• Bachelor's degree in human behavioral sciences such as human services, social work, psychology, or public health.
• 5+ years of relevant LTSS experience or combination of education and experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and travel up to 75% within assigned territory.
• Reside locally to Lake, Chaffee, Fremont or Custer Counties, CO.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management and documentation.
• Maintain confidentiality of patient information and comply with the Employee Handbook requirements.
• Conduct quarterly monitoring contacts with Members and perform face-to-face visits at least every six months.
• Update assessment tools and care plans to reflect changes in condition or services.
• Perform annual reassessments and additional assessments as needed.
• Monitor service delivery within the Support Plan and PAR.
• Attend CMA and in-service trainings provided by the CMA.
• Ensure work complies with CMA Contract Agreement requirements.
• Refer suspected mistreatment to protective services or law enforcement when needed.
• Take on special assignments and assist with system assessment and development as required.
• Travel between locations; valid Colorado licensure and liability coverage required if driving.
