Job summary

UnitedHealthcare is seeking a dedicated LTSS Case Manager to support Home and Community based services by performing intake, assessments, and ongoing case management within the assigned territory.

The role involves up to 75% travel, local to Mesa County, CO with remote work flexibility and adherence to UnitedHealth Group's Telecommuter Policy. You will participate in case conferences and ensure confidentiality and compliant documentation.

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of LTSS experience or combination of education and experience.
• Proficient with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 75% within territory.
• Local to Mesa County, CO.

Responsibilities

• Intake/screening/referral, assessment/reassessment, and development of support plans.
• Monitor Member's health, welfare, and document activities in the prescribed system.
• Conduct quarterly and semi-annual in-person contacts and update care plans accordingly.
• Complete annual reassessments and additional assessments as needed.
• Travel between locations; comply with CMA contracts and confidentiality rules.
