Job summary

UnitedHealth Group's UnitedHealthcare is seeking a Case Manager for LTSS to support patients at home and in long-term care settings, conducting intake, assessments, and ongoing care planning across the Lake, Chaffee, Fremont and Custer Counties region with occasional travel.

The role emphasizes confidentiality, quarterly and six-month contacts, and collaboration with supervisors; travel up to 75% in territory, remote work option for residents within specified counties, and adherence to CMA

Qualifications

• Bachelor’s degree in human services, social work, psychology, or public health.
• 5+ years of relevant LTSS experience.
• Combination of education and experience allowed.
• Access to reliable transportation and ability to travel up to 75% within assigned territory.
• Reside locally to Lake, Chaffee, Fremont or Custer Counties, CO.

Responsibilities

• Intake/screening, assessment, development of support plans, ongoing case management, monitoring of member health and welfare in prescribed system; documentation and resource development.
• Maintain confidentiality per policy and ensure confidentiality of patient information.
• Contact the member at least quarterly or more as warranted; conduct face-to-face contacts at least every six months and update assessments accordingly.
• Complete new assessments during face-to-face reassessments annually or as needed; update care plans as condition changes.
• Monitor service delivery per the Support Plan and PAR.
• Attend CMA agency training and in-service sessions.
• Ensure work complies with CMA Contract requirements and is timely and complete.
• Refer suspected abuse to protective services or authorities immediately.
• Take on special assignments and assist with system assessment and development as needed.
• Travel between locations; driving may be required with CO licensure and insurance.
