Job summary

UnitedHealthcare is seeking a Long-Term Services and Supports Case Manager to conduct intake, assessments, and care planning for members in homes or long-term care settings. 75% travel within assigned CO territory with the option for remote work where permitted.

Responsibilities include monitoring service delivery, updating plans, and ensuring compliance with CMA contracts. Requires a bachelor's degree in a human behavioral science field or 5+ years LTSS experience.

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of relevant LTSS experience.
• Combination of education and experience acceptable.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of health and welfare, documentation in the prescribed system.
• Maintain confidentiality of patient information and HIPAA compliance.
