Job summary

UnitedHealthcare is seeking a Licensed LTSS Case Manager to conduct intake, assessments, and ongoing case management for members in various settings. You will develop and monitor care plans, ensure documentation accuracy, and support confidentiality in all interactions.

The role requires travel up to 50% within the assigned territory, including home visits and occasional in-office work. A Bachelor's in human behavioral sciences and LTSS experience are preferred.

Qualifications

• Bachelor's degree in human behavioral sciences or equivalent.
• 5+ years of LTSS experience.
• Combination of education and experience accepted.
• 1+ years of MS Office experience.
• Access to reliable transportation and travel up to 50% of time.
• Reside locally to Moffat County, CO and hold a valid driver's license.

Responsibilities

• Intake, assessment, care plan development and ongoing case management.
• Document contacts and activities in the prescribed system.
• Monitor health and welfare and support plan delivery.
• Conduct quarterly and annual face-to-face contacts as required.
• Ensure confidentiality and adherence to CMA contracts.
• Attend CMA training and in-service sessions.
