Job summary

UnitedHealth Group is seeking a field-based LTSS Case Manager to perform intake, assessments, care planning, and ongoing case management. The role involves home visits and work across long-term care settings, with the flexibility for hybrid in-office and remote work in Colorado’s Moffat County area.

Qualifications

• Bachelor's degree in human behavioral sciences (e.g., human services, social work, psychology, public health) or equivalent experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 50% within assigned territory.
• Residence locally to Moffat County, CO or surrounding areas.
• Driver's License and access to reliable transportation.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of member health and welfare, documentation in prescribed system, resource development, and case closure.
• Maintain confidentiality of patient information per policy and law.
• Conduct quarterly monitoring contacts and face-to-face contacts at least every six months or more often as needed.
• Update assessments and care plans in documentation systems to reflect changes.
• Complete new assessments during reassessments annually or as needed.
• Monitor service delivery within the Support Plan and PAR.
• Attend CMA trainings and internal staff development sessions.
• Ensure work aligns with CMA contract requirements: timely, accurate, complete.
