Responsibilities

• Manage intake/screening/referral, assessment, and development of support plans and ongoing case management.
• Maintain confidentiality of patient information and documentation.
• Conduct quarterly and periodic member contacts and face-to-face reassessments.
• Update assessments and care plans in documentation systems.
• Travel to meet members and providers; ensure timely service delivery.
