Job summary

UnitedHealthcare is seeking a LTSS Case Manager to provide intake, assessment, and ongoing case management for members in Colorado. The role involves home visits or facility visits, with travel required to engage members and coordinate care.

You will collaborate in case conferences, maintain confidentiality, and ensure documentation is updated in the prescribed system. A combination of field, in-office, and remote work is possible near Moffat County, CO.

Qualifications

• Bachelor's degree in human behavioral sciences such as human services, social work, psychology, or public health
• 5+ years of LTSS experience
• Combination of education and experience
• 1+ years of experience with MS Office (Word, Excel, Outlook)
• Access to reliable transportation and ability to travel up to 50% within assigned territory
• reside locally to Moffat County, CO or surrounding areas
• Driver's License and access to reliable transportation

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, on-going case management, monitoring of the Member's health and welfare, documentation of contacts and case management activities
• Maintain confidentiality of patient information as described in policy
• Conduct monitoring contacts as prescribed by the Department
• Face-to-face contact with the Member at least every six months and update assessments/care plans
• Complete new assessments during face-to-face reassessments annually
• Monitor delivery of services within the Support Plan and PAR
• Attend Department-provided CMA training and in-service sessions
• Ensure work aligns with CMA Contracts and is accurate, timely, complete
• Refer suspected abuse to protective services or law enforcement
• Possibly take on special assignments and traverse system assessments
• Travel required between locations, with Colorado licensure and insurance requirements

Education

Bachelor's degree in human behavioral sciences such as human services, social work, psychology, or public health

5+ years of LTSS experience
