Job summary

UnitedHealthcare is seeking a Case Manager specialized in Long Term Services and Supports (LTSS) to provide comprehensive in-home and facility-based care management. You will conduct intake, assessments, care planning, and ongoing monitoring, ensuring confidentiality and compliance with policy.

75% travel within assigned territory, including in-home visits and coordination with providers. Hybrid work options in CO may be available for the right candidate.

Qualifications

• Bachelor's degree in human behavioral science fields or 5+ years LTSS experience
• 1+ years of MS Office experience (Word, Excel, Outlook)
• Access to reliable transportation and ability to travel up to 50% within assigned territory
• Reside locally to Moffat County, CO or surrounding areas
• Driver's License and reliable transportation

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of the Member's health and welfare, documentation of contacts and case management activities
• Maintain confidentiality of patient information and adhere to policies
• Conduct monitoring contacts as prescribed by the Department and communicate findings
• Perform face-to-face contacts with Members at least every six months and update assessments and plans accordingly
• Complete new assessments during face-to-face reassessments annually or as required
• Monitor service delivery within the Support Plan and PAR
• Attend CMA agency training and internal staff development sessions
• Ensure work complies with CMA Contract Agreement requirements and quality standards
• Refer potential abuse/mistreatment to protective services or law enforcement when suspected
• Take on special assignments and assist with system assessment and development as needed
• Travel is required; may drive and must meet CO licensure and liability insurance requirements
