Job summary

UnitedHealthcare is seeking a Case Manager for LTSS to support members in the Colorado region, including Moffat County. The role involves field visits to homes and long-term care facilities, assessments, care planning, and ongoing support.

The position requires travel up to 50% within the assigned territory, strong organization, and confidentiality. Benefits include a comprehensive package, with hourly pay between $27.00 and $40.00 based on full-time status.

Qualifications

• Bachelor's degree or 5+ years LTSS experience
• 1+ years of MS Office experience (Word, Excel, Outlook)
• Access to reliable transportation and ability to travel up to 50% within territory
• Local residence near Moffat County, CO or surrounding areas
• Driver's license and access to reliable transportation

Responsibilities

• Provide intake, screening, referral, assessment, and development of support plans and ongoing case management
• Maintain confidentiality and comply with the Employee Handbook and CMA requirements
• Conduct quarterly contacts and face-to-face visits every six months or as needed
• Update assessments and care plans in prescribed systems reflecting changes in condition
• Complete annual reassessments and monitor service delivery and PAR compliance
• Attend CMA trainings and department-led in-service sessions
• Travel between locations to support member care under Colorado LTSS program requirements
• Adhere to CMA contract requirements and maintain timely, accurate work
