• Provides resource support to members and families for employment, housing, and independent living needs.
• Monitors care plans and member status, and offers recommendations based on needs.
• Interacts with long-term care providers to ensure member needs are met.
• Collects, documents, and maintains member information to ensure regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Provides education to members and families on procedures, options, and benefits.
• Offers feedback to leadership to improve quality and cost-effective care.
