Job summary

Centene Corporation is seeking a Care Management professional to assist in developing, assessing, and coordinating holistic care management activities for long-term care members. The role includes educating members and families, coordinating with providers, and ensuring compliant care plans with 50% remote work and 50% local travel within Illinois.

The position requires a Bachelor's degree and relevant experience; IL Plan criteria include RN licensure or advanced human services education.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience or equivalent.
• Illinois RN license or Bachelor/Master’s in human services field is preferred.
• LPN with one year experience in comprehensive assessments and services for the elderly qualifies under Illinois plan.
• Experience with HCBS waiver populations is a plus.

Responsibilities

• Evaluates the needs of the member, resources available, and recommends or facilitates the plan for the best outcome.
• Assists with developing ongoing long-term care plans and identifies providers, specialists, and community resources.
• Coordinates between the member/family/caregivers and care team to ensure services are accessible.
• Provides resource support to members and families for employment, housing, independent living, and related services.
• Monitors care plans, member status, and outcomes; provides recommendations based on needs.
• Interacts with long-term care providers and partners to ensure needs are met.
• Collects and maintains member information to ensure compliance with regulations.
• May perform on-site visits to assess needs and collaborate with providers.
• Educates members and families on procedures, options, referrals, and benefits.
• Provides leadership feedback on improving quality and reducing costs.
• Performs other duties as assigned.
• Complies with all policies and standards.
