Qualifications

• Requires a Bachelor's degree and 1 year of related experience or equivalent experience.
• Illinois Plan: RN license in Illinois; Bachelor/Master in human services; or LPN with 1 year experience in comprehensive assessments and service provision.

Responsibilities

• Evaluates member needs and resources to plan optimal outcomes.
• Develops long-term care/service plans and identifies providers or resources.
• Coordinates between member/family and care provider teams for service accessibility.
• Provides resource support for employment, housing, independent living, and related areas.
• Monitors care plans and outcomes; offers recommendations based on needs.
• Interacts with long-term care providers to ensure member needs are met.
• Collects and maintains member information to ensure regulatory compliance.
