Job summary

Qualifications

• Requires a Bachelor's degree and 1 year of related experience or equivalent.
• Illinois plan adds criteria: RN license in Illinois or Human Services degree, or LPN with relevant experience.

Responsibilities

• Assist in developing, assessing, and coordinating holistic care management activities.
• Conduct in-home member visits and support PD/ Elderly Waiver programs.
• Evaluate member needs and resources to plan outcomes.
• Coordinate between member, family, and care team to ensure services are accessible.
• Provide education to members and families on procedures, services, and benefits.
• Monitor care plans and outcomes; report recommendations to leadership.
• Maintain member information to ensure regulatory compliance.
