Job summary

Centene Corporation is seeking a Care Manager to assist in developing and coordinating holistic care management activities for members in the Aurora, Naperville, and West Chicago areas. The role involves 50% local travel and a mix of home-based remote work, focusing on long-term care planning and education for members and families.

The ideal candidate has case management experience with waiver populations, strong care coordination skills, and knowledge of community resources.

Qualifications

Requires a Bachelor's degree and 1 year of related experience or equivalent experience.
For Illinois Plan: RN license in Illinois or a Bachelor/Master’s in a human services field; LPN with relevant experience may also qualify.
Must reside near the service area and be available for 50% local travel, with remote work otherwise.

Responsibilities

Evaluates member needs, resources, and plans for the best outcomes.
Develops long-term care plans and identifies providers and community resources.
Coordinates between member/family and care providers to ensure services are accessible.
Provides education to members and families on procedures, services, and benefits.
Monitors plans, member status, and outcomes, providing recommendations as needed.
Interacts with long-term care providers to ensure member needs are met.
Collects and maintains member information to ensure regulatory compliance.
May perform on-site visits and educate on options and referrals.
