Job summary

Molina Healthcare is seeking a field-based Care Coordinator in Naperville, IL to support care management and LTSS activities. You will work with a multidisciplinary team to coordinate member care, develop and monitor care plans, and address barriers to care.

Responsibilities include conducting assessments, facilitating waiver enrollment, and collaborating with care teams to ensure quality, cost-effective outcomes. Travel up to 40% may be required depending on state contracts and field needs.

Qualifications

• At least 2 years of health care experience, including LTSS exposure.
• Clinical licensure and/or certification required if state contract mandates.
• Bachelor's degree in a health care related field may be required.
• Valid driver's license, reliable transportation for job travel.
• Demonstrated knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office and online portals.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and support network.
• Monitors care plan effectiveness and documents progress.
• Promotes integration of LTSS, behavioral health, and community resources.
• Assesses medical necessity and authorizes waiver services.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Travel 25-40% locally as required.
