Qualifications

• Requires at least 2 years in health care with care management experience.
• Must have experience with LTSS or related settings.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with member, caregivers and clinicians.
• Monitor care plan effectiveness and adjust as needed.
• Coordinate services across behavioral health and LTSS.
• Assess medical necessity and authorize services.
• Lead or participate in ICT meetings for approvals.
• Provide education and motivation to members.
• Travel 25-40% as required by state/contractual needs.
