Qualifications

• Minimum 2 years of health care experience including LTSS exposure.
• Clinical licensure and/or certification required only if mandated by state contract or regulations.
• Valid driver’s license and reliable transportation for travel.
• Knowledge of community resources and ability to work with diverse populations.
• Experience in care management or medical/behavioral health settings preferred.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits if needed.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and support network.
• Monitor and evaluate care plan effectiveness and document outcomes.
• Coordinate services across LTSS, behavioral health, and community resources.
• Identify barriers to care and address psycho/social, financial and medical obstacles.
