Job summary

Molina Healthcare is seeking a Care Coordination/LTSS specialist to support care management for high-need members in Naperville, IL. You will collaborate with an interdisciplinary team to coordinate care across LTSS, behavioral health, and community resources, while maintaining focus on member outcomes and cost-effective services.

Your role involves conducting home visits, developing service plans with input from physicians and family, and monitoring progress to adjust interventions as needed.

Qualifications

• At least 2 years of health-care experience, including LTSS experience.
• Valid driver’s license and reliable transportation for state/local travel.
• Knowledge of community resources and ability to work with diverse populations.
• Ability to work independently with minimal supervision and manage multiple projects.
• Proficiency in MS Office and online portals/databases.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers and health professionals.
• Monitor and evaluate care plan effectiveness and adjust as needed.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate interdisciplinary care team meetings for service decisions.
• Communicate with members to educate and motivate changes.
• Local travel may be required (25-40%).
