Job summary

Molina Healthcare is seeking a Care Coordinator in Illinois to support LTSS members through comprehensive assessments, care planning, and coordinated services. You will work with a multidisciplinary team to ensure high-quality, cost-effective member care.

The role requires experience with disability/ LTSS populations, strong communication, and the ability to travel regionally as needed. A clinical license or certification may be required in some states.

Qualifications

• At least 2 years of health care experience with LTSS/ disability focus.
• 1 year in care management or medical/behavioral health setting.
• Licensure/certification as required by state contract or regulation.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care teams.
• Monitor care plans and adjust interventions to achieve goals.
• Coordinate services across behavioral health and LTSS.
• Identify barriers to care and arrange support services.
• Travel 25–40% as required by state contracts.
